How it works
Lip filler adds shape, definition and volume to the lips using hyaluronic acid, a molecule your skin already makes. At Serene Rejuvenation in Surrey, BC, every lip filler treatment is planned and injected by Dr. Sundeep Sarai, a physician, using the Belotero® range made for the lip.
The aim is a lip that looks like yours on a good day. That means a border that is defined rather than drawn on, an upper and lower lip in proportion to each other and to the rest of your face, and movement that is still soft when you talk and smile. Results are visible immediately and settle over about two weeks.

What lip filler can do: shape, balance and volume
People ask for lip filler for different reasons, and the reasons decide the plan. Some want volume in a lip that has always been thin. Some want a border that has softened with age redefined, or the fine vertical lines above the lip smoothed. Many simply want the upper and lower lip to match, or the two sides to sit level. Each of those needs the product placed in a different place, and treating them all as one request for “bigger lips” is how lips end up looking done.
Proportion is judged against your own face, not a template. A lip that suits a narrow face is different from one that suits a broad jaw, and a male lip is generally flatter and less projected than a female one. Dr. Sarai assesses the whole lower face before deciding where a small amount of product will make the most difference, and she will tell you if she thinks less is the better plan.
Why Belotero® for the lip
The lip wants a softer gel than the cheek or jawline does. A firm, structural filler placed in a lip can distribute unevenly and feel lumpy when you press it, so the clinic uses the softer Belotero® gels designed for the lip and its border. They integrate with the tissue rather than sitting inside it as a shape, which is what keeps the result soft when you speak.
Hyaluronic acid is also chosen because it is reversible. If you are unhappy with a result, or if filler from another clinic has migrated above the lip line, it can be dissolved with a hyaluronidase injection. That is a real safety margin, and one reason Dr. Sarai does not use permanent lip materials.
Your appointment: assessment, numbing and the injecting itself
Your lip filler appointment is booked for a full hour. The injecting takes a small part of that; the rest is assessment, photographs and planning, because Dr. Sarai would rather take her time than work to the clock. The lips are numbed before treatment, and because she is a physician she can also offer a dental block, the same small local anaesthetic injection you would receive at the dentist, for people who find lip injections hard to tolerate.
Expect swelling, and expect it to look like too much at first. Lips swell more than any other filler site, and the first two or three days are not the result. Two weeks is the point at which the outcome can be judged, and asking for more before then is a common regret. If you have ever had a cold sore, say so at your consultation: injecting the lip can wake that virus, and a short course of antiviral medication beforehand substantially reduces the risk.

How long does lip filler last?
Lip results are shorter-lived than filler placed deeper in the face, because the lips are in constant motion. Six to twelve months is typical, and many people book a smaller top-up rather than repeating the full amount. How long yours lasts depends on the product, the amount and how quickly your body breaks it down.
Will my lips look natural?
That is the aim of every lip treatment here. Dr. Sarai plans for proportion first and volume second, places less than you might expect, and reviews before adding more. Lips that look done are usually the result of too much product, too firm a gel, or filler placed outside the border, and all three are avoidable.
Does lip filler hurt?
The lips are sensitive, so they are numbed before treatment. Most people describe pressure and brief pinches rather than sharp pain. For people who find lip injections difficult, Dr. Sarai can also provide a dental block, which numbs the whole area the way a dental visit does.
How much swelling should I expect?
More than at any other filler site. Lips can look noticeably larger for the first two or three days, sometimes unevenly, and light bruising is common. Cold compresses help. Judge the result at two weeks, not two days.
What is the difference between lip filler and a Botox® lip flip?
Lip filler adds volume and shape with hyaluronic acid. A lip flip uses a small amount of Botox® or Xeomin® to relax the muscle above the upper lip so slightly more of it shows when you smile, without adding volume. A lip flip is subtle and lasts a few months; filler is the treatment for shape and fullness. Some people suit both.
Can lip filler be dissolved?
Yes. Hyaluronic acid filler can be broken down with a hyaluronidase injection if you are unhappy with the result, or if filler has migrated above the lip line. Dr. Sarai offers this for filler placed at Serene and elsewhere.
What should I avoid before and after?
For a few days beforehand, avoid alcohol and, if your own doctor agrees, medications that thin the blood, to reduce bruising. Afterwards, skip strenuous exercise, saunas and hot yoga for a day, and avoid pressing on or massaging the lips unless asked to. We give you written aftercare before you leave.
